Box 5046, Des Plains, IL 600617-5046, in the amount of Six Thousand Five Hundred Twenty Six Dollars and Eighty Cents ($6,526.80) in settlement of a claim for damage to a vehicle from City of Pittsburgh Public Works vehicle, charging same to Code 11101.108000.58.58105.2021.
