WHEREAS, since 1980, Family Hospice & Palliative Care (Family Hospice), a non-profit organization serving the residents of the City of Pittsburgh & Allegheny County, provides hospice care to more patients in the region than any other hospice, and is the leading hospice provider in Western Pennsylvania; and
WHEREAS, Family Hospice is committed to quality patient care, caregiver support and education by collaborating with the area's largest staff of Board-certified palliative care physicians; and
WHEREAS, Family Hospice currently serves approximately 3800 patients a year residing within the City of Pittsburgh, Allegheny County and Western Pennsylvania, through innovative programming such as massage and pet therapy, reiki (hands on healing), expressive art and music and the Quality of Life Program; and
WHEREAS, national research shows that, historically, the most underserved populations in end-of-life care are African-Americans, and in response, Transitions was formed to help African-American families through attention to cultural sensitivity, to enroll seriously ill loved ones in services that will alleviate suffering, and provide caregivers at home with the support they need; and
WHEREAS, Transitions was developed as a collaborative partnership among Family Hospice, Bidwell United Presbyterian Church and The Northside Christian Health Center, to increase the understanding of and access to hospice services for African-American families residing throughout Pittsburgh's Northside neighborhoods; and
WHEREAS, on Saturday, June 1, 2013, community members will gather at Anderson Manor in Manchester, for Transitions' Open House & Community Resource Fair, to launch this new program;

NOW, THEREFORE, BE IT RESOLVED that the Council of the City of Pittsburgh does hereby recognize the important work and contributions that Family Hospice & Transitions provide on a daily basis, and does hereby declare June 1, 2013 to be “Transitions Open House Day” ...
