Title: WHEREAS, the Living at Home Program was established by UPMC in 1987 as a community based three year demonstration project designed to provide free medical and social services to frail elderly adults to help them maintain their independence ; and, WHEREAS ; at the inception of the program there were 4 neighborhoods and 75 clients receiving services and currently, the Living at Home Program services 14 neighborhoods and over 600 clients; and, WHEREAS, UPMC's Living at Home Program makes referral for a range of services such as home delivered meals, help with grocery shopping, medication assistance, housekeeping and yard work as well as the reassuring phone call or visit so that older adults can live as independently for as long as possible; and , WHEREAS, the Living at Home Program of UPMC is one of 2 such projects of its kind in the country and the only one to focus on case management , has a staff of 13 serving 600 clients, age 71 to 101and has demonstrated its ability to ...

THEREFORE BE IT RESOLVED, that the Council of the City of Pittsburgh does hereby honor the important work of the UPMC Living at Home Program and recognizes its 20th Anniversary with a celebration at Shadyside Hospital on September 14, 2007 ; and,
 
BE IT FURTHER RESOLVED, that the Council of the City of Pittsburgh does hereby declares September 14, 2007 to be Living at Home Day in the City of Pittsburgh  in the City of Pittsburgh.
