Be it resolved by the Council of the City of Pittsburgh as follows:

Section 1.                     Attached form “Stop the Violence Grantee Evaluation and Monitoring Report Form” is approved by Council.

Section 2.                     The attached form “GVI Support and Outreach Evaluation Monitoring Report Form” is approved by Council.
