Be it resolved by the Council of the City of Pittsburgh as follows:
 
Section 1.  The Mayor and the Director of the Department of Personnel and Civil Service Commission, on behalf of the City of Pittsburgh, are hereby authorized to enter into a Contract or Contracts, in a form approved by the City Solicitor with : PALMA KOKOWSKI REHAB SERVICES shall engage in nurse case management services in connection with the monitoring of certain worker's compensation claims and medical treatment for an eight (8) month period beginning on May 1, 2005, at a cost not to exceed $ 127,256 (One Hundred Twenty Seven Thousand, Two Hundred Fifty Six Dollars) chargeable and payable from Code Account 44-9, Workers' Compensation Program Medical/Administration, Index Code 004499, Account 574300, Fund 1000, Organization 999300, Sub Class 040, and Budget Year 2005, Department of Personnel and Civil Service Commission.
