Title: NOW, THEREFORE BE IT RESOLVED that the Council of the City of Pittsburgh does hereby commend the UPMC Living-at-Home Program for recognizing the needs of the growing senior population who are aging in place and for providing important and relevant services for seniors to live independently and congratulates the Living-at-Home Program for 25 successful years; and, BE IT FURTHER RESOLVED, , that the Council of the City of Pittsburgh does hereby declare Friday, September 28, 2012 to be “UPMC LIVING-AT-HOME PROGRAM DAY” in the City of Pittsburgh.

WHEREAS, the Living-at-Home Program of UPMC is a geriatric care coordination program that provides services for ongoing in-home care of older adults; and,
 
WHEREAS, the Living-at-Home Program makes referrals for a range of services for seniors such as home-delivered meals, grocery shopping, housekeeping and yard work to help older adults live independently for as long as possible ; and,    
 
WHEREAS, the Living-at-Home Program serves seniors at least 70 years of age and who live in Greenfield, Hazelwood, Hill District, Oakland, Squirrel Hill, Bloomfield, Lawrenceville, Shadyside, Mt. Oliver, Arlington, Allentown, Knoxville, Beltzhoover or the South Side.  Qualified seniors provide a health history, medication history and discuss functional limitations and from that, the Care Coordinator determines the kind of services that a senior will need to continue to live independently ; and,
 
WHEREAS, this year, the UPMC Living-at Home Program will celebrate its 25th Anniversary and will honor its success at a Luncheon at Shadyside Hospital's Herberman Conference Center on Friday, September 28, 2012  ; and,
